Certification Application Submission
ElitePetz Certification Authority accepts applications for evaluation under its published Certification Standards Framework.
Submission of this form constitutes formal entry into the Authority's record system.
Approval is not guaranteed.
Scope of Certification
Certification applies to entities operating within the companion animal breeding sector and within the defined scope of the Authority.
The Authority evaluates applications solely against published Standards and Governance documents.
Certification does not:
➤ Replace government licensing
➤ Constitute legal authorization
➤ Guarantee commercial outcomes
➤ Serve as regulatory approval
Certification status is limited to the Authority's registry.
Review Process
Upon submission:
➤ The application is logged and assigned a record identifier.
➤ A completeness review is conducted.
➤ Substantive evaluation is performed.
➤ A formal determination is recorded in the Status History system.
The Authority does not provide advisory opinions or preliminary eligibility assessments prior to formal determination.
Applicant Responsibility
The applicant bears full responsibility for:
➤ Accuracy and completeness of submitted information
➤ Ongoing compliance with applicable Standards if certified
➤ Timely notification of material changes
Misrepresentation may result in denial, suspension, or revocation.

Certification Fee
Certification is issued upon successful review and payment of the certification activation fee.
Early Certification Fee (Founding Applicants): $199.00
This fee covers administrative review, certification issuance, and registry maintenance.
I hereby declare that the information submitted is accurate and complete to the best of my knowledge. I confirm that I have read and agree to the Certification Applicant Agreement, Certification Standards, Code of Ethics, Terms of Use, and Privacy Policy of the ElitePetz Certification Authority.
